Swelling or Lymphedema (edema) from any cause can become severe, limit mobility, and be painful if left untreated. Initial treatment for significant edema may involve Compression Bandaging (also known as multi-layered lymphedema bandaging). This treatment helps move fluid from the extremity back to the core. It will reduce swelling and return limbs to a more normal shape. This is a highly specialized bandage system and application (and not simply wrapping the leg with an ACE bandage!). Several layers, special padding, and protective techniques will be utilized to achieve the maximum effect.
As long as you tolerate the bandages, they should initially be worn day and night (without taking them off). You will achieve the best effect if you walk and continue your daily activities. You may be expecting the bandages to feel uncomfortable, but most people report the bandages actually decrease their leg discomfort. At first, while your swelling is rapidly decreasing, you will need to come to the clinic every 1-3 days to have the bandages changed (or you will change them at home after we teach you how!). The reason they must be changed frequently at first is that as your swelling decreases the bandages become loose (and that’s a great thing!).
Compression bandages are usually applied after lymphatic massage is performed on your limb to help move fluid toward the core where it can be reabsorbed by your circulatory system. Lymphatic massage and compression therapy work well together and are very effective at decreasing edema.
We will often utilize and/or prescribe special compression pumps – that feel like a gentle massage – to aid in moving the extra fluid in your limb toward your core. This is another way we reduce the swelling in your leg.
Once your limb is closer to its normal size, you will graduate from compression bandages to a compression garment. This is a garment made to fit your limb and control swelling. It’s important to continue to wear it every day. In some cases, a nighttime garment is needed as well. Your provider will know and prescribe the best treatment for your condition.
It’s important to understand that some types of lymphedema will require life-long treatment. You may need various types of compression garments through out your life. While there are many types and qualities of compression stockings that can be bought “off the shelf” or online, significant lymphedema will likely require highly specialized garments that are made to fit your specific measurements. Not all clinicians are qualified to prescribe or measure for these garments. Make sure your clinic has a clinician who is (hint: we do!).
